If your student has medications at school, please note:
-No medications will be sent home with students.
-You may pick up the medications Monday-Friday 8am-3pm the last week of school.
-You may also pick up medications before or after the Awards Ceremony.
-Any medications not picked up by the last day of school will be disposed of.
2015- 2016 Indiana State Requirements for Immunizations
I want you to know what immunizations are required for next school year. This way, you may be able to take care of these at MD appointments over the next few months, without having to make an unnecessary trip. Plus, doctor’s offices will book quickly with everyone trying to get this done before the start of school! I recommend printing this out and taking it with you to your appointment.
3-5 year olds: 3 Hep B, 4 DTaP, 3 Polio, 1 MMR, 1 Varicella
K-1st grade: 3 Hep B, 5 DTaP, 4 Polio, 2 MMR, 2 Varicella, 2 Hep A
